For your convenience, you can now download all of our NEW PATIENT forms from our website, fill them out ahead of time and fax them to our office at 505-792-5771. If you don’t have access to a fax machine, just bring them with you to your appointment completely filled out (please!).

In the meantime, please try to fax the forms to our office at least 1 day ahead of your first dental appointment so that we can have the time needed to obtain the most up-to-date insurance benefit coverage for you and provide you accurate estimates for the cost of your dental care.
